Add Host field to the queue.Request struct and copy it from Host field
of the HTTP request when converting HTTP request to queue.Request in
MakeQueuedProxy function.

Note, not all `alexellis/github` references should be changed, there are
a number of repos which are not part of the openfaas org, this commit
excludes those.
